Achilles tendinitis, tendinosis
A tendinitis / tendonitis lesion of the achilles tendon is very common : mostly the lesion lies at the medial/lateral aspect of the achilels tendon in combination with the anteromedial or anterolateral part. A lesion at the insertion is possible too, but is rather rare. Palpation will be conclusive. Deep transverse friction massage in combination with eccentric exercise therapy is a good treatment strategy. In exceptional cases an infiltration could be useful. Mostly twee different friction massage techniques need to be combined. (Part I, p154-155 ; Part II, p35-36)
Achilles tendinosis (2) Different stages are described :
stage I pain during and after walking/running stage II pain at the start, better during activity and worse again after exertion stage III severe pain during exertion stage IV pain also at rest.
Remark : DF can also be applied on nodular scar formation after an old partial rupture or after surgery, with excessive scar formation (perhaps 2-3 months, twice a week treatment).
Achilles tenovaginitis (rheumatoid variant)
There is a contrast between symptoms and signs : the tendon is warm, swollen, very tender on palpation, possibly with nodes in it ; the functional test, however, is only slightly painful. Since this is a rheumatoid variant, no DF will be used.
