Cuboid rotation

Subluxation of the cuboid bone in athletes, caused by too much traction of the peroneus longus tendon (the lateral border of the cuboid moves upwards and the medial border downwards ; it stays fixed in this position). This seems to occur in 4% of all foot problems seen in athletes.

The pain is lateral during and after activity ; the mid-tarsal tests are painful, not limited, and movement is seen to occur only at the inner part of the foot. The plantar cuboid ligaments are tender on palpation.


Treatment : manipulation of the cuboid bone in a dorsal/lateral direction with the patient prone lying or standing.


Remark : a reverse situation, whereby the lateral border of cuboid has moved in plantar direction, is sometimes seen in an inversion trauma. This can be manipulated too : we take a contact on the dorso-medial part of cuboid and manipulate in plantar-medial direction.
