This is an occlusion of the tibial artery, due to increased muscular activity in a restricted space. The anterior compartment, the space between tibia, fibula, interosseous membrane and the fascia around the muscles, is too small.
Tight fascial compartment (2) During activity muscle volume increases up to 20%. Claudication-like symptoms occur. What are the differences with intermittent claudication ? In claudication the symptom is pain ; in a tight fascial compartment, it is weakness after 30 minutes walking. In claudication the pain disappears in 1-2 minutes if the patient stops walking ; in a tight fascial compartment it can take half an hour before the full muscle strength has come back.
An acute form exists, manifesting itself in strong pain, redness, warmth and swelling ; it needs to be surgically repaired immediately. If not, it might lead to necrosis of the tibialis anterior muscle.
Remark : swelling also can cause pressure on the n. peroneus superficialis, resulting in pins and needles at the lateral boarder of the foot.
